Case Care Coalition
Case Care Coalition case managers serve as the primary point of contact for youth care, connecting them to the various services they need and are eligible for.
We understand that every adolescent is unique and requires personalized care. We also know how challenging it can be to navigate the complex web of services young adults may require. Our case managers work to coordinate tailored care, ensuring that all needs are met and identifying any gaps or areas where services may fall short.
15-18 grades 9th-12th Parent or guardian must be present during intake consultation. Meet with the parents for the first session. Going forward meet once a month for 1 hour; severe cases will meet each month until progress is shown within data.
Each session will include a detailed report explaining the stance of the client and the next step of moving forward by using a growth plan demonstrating a timeframe to handle the unique problem.

Case management ensures that individuals receive the right services at the right time. It involves organizing and connecting various resources to meet the client’s unique needs, making sure they don’t fall through the cracks or miss out on essential support.
